Below are the terms and conditions for you to cancel your membership.

Before you cancel your membership, let's explore other ways to still provide the services you signed up for.

1. We can give you private lessons for the time missed 2 lessons for every month missed.
2. We will give you a credit for time missed. Up to 2 months credit.
3. Any gear, or uniforms needed will be provide for the equal value of time missed.

We are sorry to hear that you wish to cancel your membership with New Journey MMA.

We respect your decision or reason and want to ensure the process is as smooth as possible.

By signing this cancellation notice, you acknowledge and agree to the following:

☆ Cancellation Request: I wish to cancel my membership with New Journey MMA effective upon the first mention of cancel which needs to be done in writing.

☆ Sound Mind and Health: I confirm that I am canceling my membership voluntarily and in sound mind, health, and body. I understand that this decision has been made without undue influence or pressure, and I(child/children/adult) am in good physical and mental condition at the time of cancellation.

☆ Reminder: All memberships require 15 - 30 day's notice before the next billing cycle to cancel.

☆ Outstanding Fees: I agree to pay any outstanding fees or dues up until the effective cancellation date (see membership form).
